Could it be allergy-related, Brenda?

Have they done a scraping for demodex mange? I knew a beagle that had a very localized patch for a very long time... It's a long shot, though, since normally the mange would spread...

What dog foods have you tried? Any with a single novel protein and carb source (like venison and sweet potato)? The R/D is pretty horrible stuff, consisting mostly of corn (common allergen), soy (also common allergen) and sawdust (which is basically what 'powdered cellulose' is). I would seriously revist the diet issue, even if it isn't directly causing the skin problem.

Ground Whole Grain Corn, Corn Gluten Meal, Chicken By-Product Meal, Powdered Cellulose 10.4% (source of fiber), Soybean Mill Run, Soybean Meal, Chicken Liver Flavor, Dried Beet Pulp, Soybean Oil, ....

Allergies to food often take time to develop, sometimes even years.
