White patch on underside of tongue?

Hey guys,

Hubby and I were playing with the nookie monster last night. He was laying on his side with his big floppy tongue hanging out and something white caught my eye. I opened his mouth and there is a big, white patch on the underside of his tongue. It is covering about half the area underneath.

I called our vet clinic this morning but she has 8 surgeries and won't be able to return my call until tomorrow. I will be calling around to find a clinic who is not too busy to return a call, but in the meantime, do any of you have any idea what this might be?

He's on medication right now:

750mg Cephalexin 2x daily (1500 mg total)
30mg Prednisone 2x daily (60mg total)
30mg Phenobarbital 2x daily (60mg total)

Was thinking maybe a yeast infection from the antibiotic?

Aww geeze, poor Nanook ....you guys just can't catch a break.

I am thinking it may be oral thrush from all the antibiotics he's been on. Or else he does have another type of fungal infection. I hope you can get to talk to a vet soon. Do you have any plain organic yogurt in the house you can give him?

A few questions,

How old is Nookie Monster?
Why is he on those medications?
Cephalexin - antibiotic, for what type of infection?
Prednisolone - steroid. what inflammatory condition?
Phenobarbitol - anti-seizure medication. Epilepsy? when did the seizures start?

I don't suppose you can take a picture of the area? I realize that is a difficult site to photograph.

Yeast infection would be unlikely within the mouth. They are common on the outside of mouths.

Thank you for responding Dr. Lee.

Nanook is 5 1/2 months

He's on the Cephalexin as he has fluid in his lungs. We had a Transtrachael flush done last week. They got the cytology report and still don't know what it is. They said there is bacteria present along with fungal spores. The vet said it may be bacterial pneumonia from aspirated during one of the seizures but then said the x-rays don't support that. He also said it could be something inhaled, enviromental etc. The fungal spores he said could be inhaled, from his oral cavity, enviromental or from aspiration. So the cytology wasn't very helpful. They are in the process of growing cultures, both bacterial and fungal. He has a cough and an occasional "rotten fish" smell to him. They have checked his anal glands and don't think there is anything wrong there. The fishy smell is on his breath. I have noticed it when he burps.

The steroid is for possible sterile mengingitis. They treated him with 50mg daily for a week, then dropped it to 10mg daily for 2 weeks. He had a seizure on the last day of the low dose so they put him on another 2 weeks of 10mg daily. He had a CSF tap which indicated possible parasitic infection or sterile meningitis. He has had serum tests for neospora/toxo/crypto, all negative.

The pheno is to help control the seizures. The last one they think he may have aspirated, so they put him on pheno as he had 2 in 4 hours. They told us he is too young for epilepsy in the beginning, but one vet is now thinking that's what is is while another, after conferring with an internal specialist, thinks it is not epilepsy but sterile meningitis.

He had his first seizure at about 2 1/2-3 months (that we are aware of).

Thank you again for responding.

He is at daycare right now, and I'm at work. Once I get home this evening I can take a picture and post it.

I know... poor little guy
I will stop and get some on the way home. Even if it isn't a yeastie, I'm sure it can't hurt him.

No, the yogurt won't hurt him ......get an organic brand that only has milk ingredients and active bacterial cultures. In fact, you could get probiotic capsules instead if Nanook doesn't like yogurt and they would have a lot more active cultures but give them a few hours apart from his meds.

I'm glad that Dr. Lee responded ......it will be great to get his expert opinion on poor Nanook's problems.

I will stop on my way home and get probiotic capsules. He LOVES yogurt, but also gets diaahrrea when we give him anything other than his food.

And yes, will be great to get his expert opinion. Fresh eyes are always welcome

Sorry it took so long to get these pics. When we got home, we checked first thing and the white was gone... we thought for sure it was the skin itself that was white last night, but apparently it's a film that comes and goes. We waited about an hour and noticed it on his tongue again, so I grabbed the monster and hubby grabbed the camera. The first pic shows there was nothing there... very odd. In the last pic there is also an orangish bubble looking thing, and I could not get a good shot behind the teeth, but it was very white towards the back. You can see it a little.

Last one. We have no clue what it is and our vet never did call us. I called 3 others, but no one wanted to talk to us and said to contact our own vet

I think you are right. And no... our vet has not called us back, which is dispicable. I have called 4 other vet clinics but they all say the same thing... they don't want to interfere with our current clinic because they have already done all the testing and know his history

You'd think there would be one who would be willing to take our baby in, if nothing else, to tell us what is in his mouth

I think I may just talk to my boss and take a few vacation days and take him to Washington.

You know, after spending thousands of$$$$ at your vet-clinic,I would bring Nanook and insist they look at him,I assume not all the vets at the clinic are doing surgery at the same time.

I thought about doing that but I don't want them to treat him badly out of spite for us

And yes, you would think after more than $8,000 (for both Thorin and Nanook, $7,000 for just Nanook) they would make the time to call me.
There are only 2 vets there. Our regular vet who works Monday and Tuesday then the owner who works Wed-Sat.

I cannot believe the way your vet is treating you. I would be hitting the roof with them by now ......they should at least have the decency to call you.

I'm at a loss, I just don't know what to do. I called a couple more vets who said the same, they won't step in (even to check his mouth) and said take him to our vet. I was tempted to just take him to one, not tell them about any of his problems and just have them check his mouth. I was afraid though that if they give him meds for it, it may react badly with what he's on. I just don't know what to do and we can't take him to Washington until next week

You can see he has blisters, that has to be at least a little painful.
Our poor little monster

So what ended up being the problem. I've noticed the same white patch on the tongue of my Labrador. We're booked in for the vest tomorrow. Just wondering what to expect.
