Unhappy cat pulling out fur, dry skin

I have a grey domestic shorthair, 12 yrs old. For the past several years, he's regularly pulled out tufts of fur from his lower back. His skin has always been dry there, but now the dry skin and fur pulling are getting worse - he's pulled out enough fur that you can see large skin flakes underneath. There's no scabbing or evidence of infection.

Of note, he has a history of stones and has eaten Hill's Prescription c/d-oxl for many years to help reduce stone recurrence.

I'm thinking of giving him a bath w/ cat dandruff shampoo, brushing him, but wonder if this is the correct approach. Appreciate you help.
