neutering complication

My 4 year Alaskan Malamute was neutered a week ago, yesteday we took him back to the vet because his scrotal sac was very enlarged. He was sent home with antibiotics as well as pain medicine and he is to go back this thursday for surgery. Upon reading other posts most say that this problem should go away on its own with the help of warm and cold compresses. Does anyone know why they would perform surgery other than maybe inserting a needle to relieve pressure?
