When you're applying ear med's to a cat, the tip has to touch the cat's ear, it has to be right inside the ear, or the med's will be noneffective because the drops most likely won't be absorbed deep...

Enema's aren't that bad, especially if they are done properly. They can be somewhat invasive, but you're pet would prefer it over not being able to have a BM at all. Good luck and keep us posted ;)

The only thing that I can think of is that there has been some sort of damage to the sphincter muscle, which would result in the anus not being able to retain stool. Good luck at the vet :)

In clinic we use gauze to muzzle dogs if we don't have a proper muzzle on hand (like the pantyhose idea). We just loop it around the dog's muzzle and then behind the ears and it works fine.
