If this is the way she filled out her application form it would be no wonder she did not get the "puppers". :D I do agree that this form is the craziest adoption form I have ever seen. Though it does show that they are serious and would scare off (or at least make them think twice) any potential animal abusers, neglecters, etc.... :D
