Take her to vet to do thorough examination of her teeth and gums and (take stool sample) and have that checked for worms and if all that comes back normal then change her food.

Yes often it is from either cheap crappy food or a food that just doesn't agree with your dog. What brand do you feed?

I would be checking for tartar buildup at the gum line - this was (is) my Eskie's problem - perfect teeth, but the tartar problem. Had teeth cleaned in Oct. and tartar is building up again, despite every single preventative measure - and he eats an excellent food as well. My vet herself has the same problem - 3 weeks after cleaning, tartar is back - just prone to it.

You could try adding something like Oxyfresh to the dog's drinking water. it helps with breath, tartar buildup and can help to prevent periodontal disease.
